Family Connections 757 mission is to provide mental health services to persons and families in order to strengthen, stabilize, and support them. Family Connections has established 5 core values that form our framework: Compassion, Respect, Integrity, Professional Growth, and Promoting Independence.

Who Do We Work With?

Adults who demonstrate significant concerns in major life activities due to a mental, behavioral, or emotional illness. Some require individualized training in acquiring basic living skills, such as symptom management, adherence to medication schedules, development and appropriate use of social skills and personal support systems, personal hygiene, food preparation, or money management.

Individual Therapy

Life may be unpredictable. Things may appear to be simple at first, but they gradually descend into pandemonium. Circumstances can build up to the point where they become uncontrolled. We are here to walk with you through difficult times in your life. Individual counseling can help people of all ages who are dealing with any of these typical concerns.

Intensive In-Home (IIH)

Intensive In-Home (IIH) is a home-based, highly supportive, and comprehensive program that provides stability for adults and their families who are experiencing a mental health crisis. By design, IIH services are more intensive than traditional mental health services as they focus on the stabilization of the adults across multiple settings with the goal of decreasing future hospitalizations or other high-level out-of-home placements.

Mental Health Skill-Building Services (MHSS)

Mental Health Skill-Building Services (MHSS) is an intensive, home-based service that is designed to assist the client in obtaining independence or stability within the community. MHSS is available for adults who have long-term mental health needs, cognitive deficiencies, or dual diagnoses. Our MHSS program focuses on 5 core competencies to promote independent living while managing mental health symptoms and needs. The 5 core competencies consist of Mental Health, Employment, Money Management, Self-Care & Daily Living, and Personal Development..
